As Therapists, we know the benefits of taking notes, particularly ‘writing’ notes. The act of physically using a pen accesses our logical brain in a way that typing perhaps does not. For many it’s a pleasure to write, so why not have a comprehensive yet aesthetic journal to use in your profession.

About This Journal
This Journal is a tool that is not only useful but beautiful…
A handy portable A5 sized notebook encased in a soft vegan leather cover with an elastic binding and ribbon to keep your page. A six-ring internal snap-open binder to keep pages stable and organiser tabs so that you can find sections in the notebook fast and precisely, plus a pen loop to keep your favourite stylo handy.
The coloured organiser tabs separate the different sections of the journal.
The alphabetical tabs help keep your client notes easy to find.
The monthly tabs help keep your client calendar bookings in order.

2. Calendar for Client Bookings
The Calendar is not pre-dated so that you may write in the dates each week that you are actually working. Bookings available are on the half hour and each entry has a payment check box for you to tick and date when your client has paid to help you keep track. There are monthly tabs for you to organise and efficiently access your bookings throughout the year.
3. Supervision Records
To keep details of your Supervision sessions and a brief overview of what was discussed. Keeping your own records will enable easy reporting should they be requested and most importantly help you to reflect over personal, professional and client progress.
4. CPD Records
To keep dates and hours recorded of professional development. Keeping these records will enable easy reporting should they be requested. Having these records makes things easier when creating a CV or searching for the original date of certificate for each training event.
5. Finance Records
Jotting down income and outgoings will help particularly if in self-employment. This section is designed to keep you savvy and financially informed as the year progresses.
6. Perforated Client Reminders
Tear out slips for you to fill-in to hand over to clients regarding – 'next appointment' and 'bank transfer' details.
7. Session Content & Process Notes
One page per session is used to record content, your observations, client progress, reflections and any homework, interventions or signposting. A notes page can be used during sessions for guidance in addition to your post session account.
Session Content Notes - These notes contain the basic details about what was covered in session plus some observations. These notes can be requested to be looked at by the client, police or a court.
Process Notes - These notes are personal thoughts, observations and feelings around how the therapy is progressing and not to be given out on request - can also aid with your Professional Supervision sessions.
8. Client Details & Initial Assessment
This is a comprehensive page reminding you to record all of your client details. It contains sections like: Brief initial issues, G.P. & medical details, emergency contact, name, address, phone number, mood/ anxiety/ stress ratings and client goals. This page is designed as a guide to have in front of you during your first appointment. It prompts you to gather a truly comprehensive client record.
Handling Confidential Data:
Confidentiality is a cornerstone of ethical guidelines in therapy. Client's shared information must remain private, so following the guidelines outlined by our Ethical Professional Bodies is essential as part of our profession. a client’s personal data should be kept separate from their session notes and ideally in a locked cabinet or additionally password protected if using digital technology.
Tips to hold data securely:
- Designate a code for each client on ‘Session Notes’ rather than using their name.
- Use an A5 alphabetical organiser to store ‘Client Details & Initial Assessment’ pages (section 8) separately and keep in a locked cabinet or similar.
(You can also staple older client notes together with their corresponding ‘Client Details & Initial Assessment’ pages in an A5 organiser to free up space in your journal).
- Alternatively, take a picture/scan of the ‘Client Details & Initial Assessment’ pages and store on a password protected device, then ideally destroy the physical record or additionally store in your organiser in a locked cabinet. This way you can safely access the details on your password protected phone/tablet/laptop at any time.
(You can also take a picture/scan of older client note pages to free up space in your journal and store in files on your laptop/tablet).
Handy Hint
Remember if you do decide to remove ‘Client Details & Initial Assessment’ (Section 8) to store separately, it’s useful to re-use section 8 in your journal for ‘Top-Up’ Client Session Notes pages instead.
Also, if your journal is getting full, take out older client notes and store together with the ‘Client Details & Initial Assessment’ page in your separate A5 expandable organiser.
